Qualitative Defects in WBC

ToxicGranulation

http://www.med-ed.virginia.edu

Toxic Granulation :

  • excessive granulation
  • response to enhanced lysosome enzyme production
  • vivid blue-black coloration
  • clusters of toxic granules appear in neutrophils

ToxicVacuolation

http://www.nejm.org/doi/full/10.1056/NEJMicm040332

Toxic Vacuolization :

  • changes in segmented neutrophils
  • vacuoles appear in the cytoplasm of this cell and may be small or large
  • occurs from :
    • prolonged exposure of blood to drugs such as sulfonamides
    • tubes held for a long period of time
    • delayed creation of smear – pseudo-vacuolization

DohleBodies

http://www.med-ed.virginia.edu

Döhle Bodies :

  • cytoplasmic inclusion composed of ribosomal RNA
  • near the cytoplasmic membrane
  • rod-shaped pale bluish-gray structure
  • may be seen in nonpathological conditions such as pregnancy

HE

Human Ehrlichiosis (HE) :

  • group of tick-borne diseases that show a notable white cell inclusion
  • human monocytic ehrichiosis
    • caused by rickettsia-like bacteria Ehrlichia Chafeensis
  • human granulocytic ehrlichiosis
    • caused by the rickettsia-like bacteria Ehrlichia phagocytophilia
    • onset of high fever, chills and headache
  • low white counts
  • elevated liver enzyme
  • mulberry-like inclusions may be seen in bone marrow granulocytes or monocytes

Hypersegmentation

http://www.eglobalmed.com

Hypersegmentation :

  • normal neutrophils have 3-5 lobes in the nucleus
  • hypersegmented contains >5 lobes
  • usually seen in megaloblastic processes
    • folic acid
    • pernicious anemia
    • vitamin B12 deficiency
